Prospective CHESF Yearbook Advertiser/Sentiment Application

Thank you for taking the time to consider supporting our organization through an advertisement or personal sentiment in the 2019-2020 CHESF Yearbook. CHESF is a 501(c)3 organization and all payments are tax deductible for IRS reporting.

     Please complete this form and follow all instructions for uploading the desired digital ad or text. You will be taken to a Paypal payment page and receive an email of your paid invoice.

     All digital ads or photos need to be camera ready copy at 300 dpi. Email them to [email protected]. Sentiments should be clearly printed in the space provided.

If you are a current CHESF member or were referred by a CHESF member, please add a name and contact information in the space below so we can give our thanks and a token of appreciation. If a member of the High School Student Council referred you, please note that 50% of the proceeds will go towards high school team building and support activities. Accordingly, please place High School Student Council, rather than the student's name, in the referral location under those cirsumstances.
